Low back pain is one of the most common complaints in golf.

That does not mean it is normal.

A lot of golfers assume back pain is just part of the game. They blame age, tight muscles, too many range balls, or a bad mattress. Others assume the problem must be their swing, so they chase technical fixes.

Sometimes the swing needs work.

But many times, the real issue is not the swing itself.

It is the body underneath the swing.

The Low Back Is Usually the Victim, Not the Driver

The lumbar spine is built for stability more than large amounts of rotation.

Golf, however, is a rotational sport.

The swing requires coordinated motion through the hips, pelvis, thoracic spine, shoulders, and feet. When those areas move well, the low back does not have to absorb excessive rotational stress.

But when the hips are stiff or the thoracic spine does not rotate well, the body still has to get the club back and through the ball somehow.

So it borrows motion from somewhere else.

Very often, that somewhere else is the low back.

That is why many golfers feel pain during the backswing, transition, or follow-through. The low back is not necessarily weak. It may be overloaded because another part of the chain is not doing its job.


The Three Biggest Movement Problems Behind Golf-Related Back Pain

There are countless reasons a golfer can develop low back pain, but three movement limitations show up again and again.

1. Limited Hip Internal Rotation

Hip internal rotation is one of the most important and overlooked movement qualities in the golf swing.

During the backswing, the trail hip needs to internally rotate as the pelvis turns away from the target. During the downswing and follow-through, the lead hip needs to internally rotate as the pelvis opens toward the target.

If either hip is restricted, the pelvis cannot rotate cleanly.

When the pelvis gets stuck, the golfer often compensates by rotating more through the lumbar spine.

That can increase stress through the joints, discs, muscles, and connective tissues of the low back.

A golfer with limited hip rotation may not feel hip pain at all. They may only feel back tightness, stiffness, or a sharp pinch after swinging.

The back gets blamed.

The hip may be the driver.

2. Restricted Thoracic Rotation

The thoracic spine, or mid-back, should contribute a significant amount of rotation during the golf swing.

If the mid-back is stiff, the golfer may struggle to complete a full shoulder turn without extending or rotating excessively through the low back.

This is especially common in golfers who sit for long periods, train with too much emphasis on sagittal-plane strength, or have never worked on true rotational mobility.

A limited thoracic spine can create several common swing and pain patterns:

Reduced shoulder turn.

Early extension.

Loss of posture.

Excessive lumbar rotation.

Overactive arms during the downswing.

Back pain after longer practice sessions.

Again, the pain may be in the low back, but the movement limitation may be above it.

3. Poor Pelvic Control

Mobility matters, but control matters just as much.

Some golfers have enough range of motion when tested passively, but they cannot control the pelvis well during the swing.

They may sway instead of rotate. They may extend through the low back instead of turning through the hips. They may lose posture during transition. They may struggle to separate the pelvis from the trunk.

This is where the difference between flexibility and usable mobility becomes important.

A golfer does not just need to “get loose.”

They need to control rotation under speed, load, and timing.


Why Rest and Stretching Usually Do Not Solve the Problem

Many golfers try the same cycle over and over.

Take a few days off.

Stretch the hamstrings.

Foam roll the back.

Maybe get a massage.

Feel better.

Play again.

Pain comes back.

The reason this cycle fails is simple: the underlying movement problem was never identified.

If the low back is irritated because the lead hip does not rotate, stretching the low back may calm symptoms temporarily, but it does not change the swing demand.

If the thoracic spine is stiff, massage may reduce tone, but it may not restore the rotational capacity needed to swing efficiently.

If the pelvis cannot stabilize and rotate under control, rest will not teach it how.

Symptom relief matters, but it is only the first step. The real goal is to change the movement pattern that keeps overloading the tissue.


What a Golf-Specific Assessment Should Include

A golf-related back pain assessment should look beyond the painful area.
